There are two ways to submit an Individual Account Application for an Infinity Pro account:

1. Secure Electronic (Online) Application

Step Two. Submit a copy of an official government photo ID (e.g., drivers license, passport) and proof of residence (e.g.: utility bill, bank statement).

2. Paper Application
Step One. Download and print the following forms (PDF format):

Step Two. Submit a completed application with a copy of an official government photo ID (e.g., drivers license, passport) and proof of residence (eg.: utility bill, bank statement).
